CCNC is a statewide program that serves 1.7 million Medicaid beneficiaries through dedicated support to primary care practices provided by local, on-the-ground clinical teams who know their communities and local resources intimately. 

The Community Care of North Carolina program is led by a seasoned management team under the direction of two Boards of Directors, one that oversees Community Care of North Carolina, Inc. (CCNC) and another that oversees North Carolina Community Care Networks, Inc. (NCCCN). NCCCN contracts with the State of North Carolina under the NC Medicaid's Primary Care Case Management program.

CCNC supports the provision of high-quality primary care through state-of-the-art predictive analytics and long-term relationships with patients, primary care physicians and a wide range of providers that together make up the medical neighborhood.
