CCNC lauded for leadership in healthcare transformation
At the September 2019 meeting of the North Carolina Institute of Medicine (NCIOM) at the McKimmon Center in Raleigh, CCNC was called out as a national leader in health care transformation. CCNC’s Transitional Care Management was touted as a “transformation strategy that works” by Families USA Executive Director, Frederick Isasi, J.D. MPH.
Isasi’s “top five” strategies are listed below. Though he calls out CCNC specifically for coordination transitions in care, all five approaches listed have been foundational in CCNC's long-term success.
