CCNC Data Briefs Detail Strategies for Reducing Avoidable Emergency Department Visits
Two just-released data briefs detail CCNC’s approach for using data to identify opportunities to impact future ED use through care management interventions targeting patients with a higher likelihood of returning to the ED. Rather than rely on traditional algorithms that focus on a select set of “avoidable” diagnoses, CCNC developed a predictive model to stratify patients based on their propensity to return to the ED after an initial visit. This “ED Risk Score” was applied to CCNC’s population to focus care management intervention efforts after ED use. For patients in the highest ED Risk strata, CCNC care managers were able to prevent one ED visit for every five patients care managed.
