CCNC Improves Quality

The cornerstone of CCNC’s approach to quality improvement is to establish a connection to a primary care medical home for each and every Medicaid recipient. A critical element to CCNC’s success centers on the ability of the networks to locally implement system changes and provider supports that are needed to improve quality in practices. Community Care of North Carolina’s provider-driven approach to quality improvement has benefited tens of thousands of North Carolinians with diabetes, asthma, hypertension, heart failure, and cardiovascular disease; and has achieved better results than commercial Medicaid managed care nationally.
