CCNC's model for community pharmacies featured in publication by Center for Health Care Strategies

A new brief by the Center for Health Care Strategies (CHCS), "Opportunities to Advance Complex Care in Rural and Frontier Areas," details CCNC's efforts to coordinate care to better meet the needs of rural complex care patients and address geographic barriers in frontier communities. 

CCNC developed the Community Pharmacy Enhanced Services Network® (CPESN®), a clinically integrated network of community pharmacies who provide patients with complex medical and social needs with enhanced medication management services. CPESN® pharmacists work hand-in-hand with primary care physicians, care managers, and behavioral health providers to improve outcomes for individuals needing a higher level of care. 

A few of the specialized services key to managing complex patients provided by CPESN® pharmacies include medication reconciliation, medication home delivery, adherence coaching, and daily dose multi-medication blister packaging.
