North Carolina Community Care Networks, Inc., the subsidiary of CCNC that manages the Medicaid population, has assumed certain obligations formerly held by the AccessCare network. CCNC Clinical Operations collaborates with primary care providers, Departments of Social Services, Mental Health and other community agencies to ensure prevention, treatment and continuity of care.
CCNC Clinical Operations serves patients in: Alamance, Alexander, Alleghany, Ashe, Avery, Burke, Caldwell, Caswell, Catawba, Chatham, Iredell, Orange, Robeson, Sampson, Watauga and Wayne.
Community Health Partners (CHP) started in July 1998 as the community based Medicaid managed care entity in Gaston. It expanded into Lincoln County in 2001. The network works collaboratively with local health departments, departments of social and human services and many other community organizations, including area hospitals. Our staff provides care management for high risk patients, disease management, utilization management, transitional care (hospital to home) and behavioral health integration. CHP programs and initiatives also include the ABCD/Early Intervention Program, Palliative care, Health Check, Care Coordination for Children and OB case management for high-risk pregnant women.
Northern Piedmont Community Care (NPCC) is a partnership between Durham Community Health Network (for Durham county) and Community Care Partners (for Vance, Warren, Person, Franklin and Granville counties) to promote wellness strengthen the self-care capacity of its Medicaid members and their families. With an interdisciplinary staff of professional and paraprofessional providers, NPCC has focused its efforts and energies on the development of community-based care/disease management that supports the integration and collaboration of the various patient delivery systems within the community. The staff of NPCC deliver at-risk social work services, service coordination, access support, education and nutrition counseling.

Northwest Community Care Network (NCCN) began in 1998 with Surry as the original pilot county. NCCN began to add additional counties in December 2001 and has grown to cover Carolina Access recipients in the counties of Davidson, Davie, Forsyth, Stokes, Surry, Wilkes, and Yadkin. The program as developed in these counties serves both rural and urban settings. In order to provide the necessary managed care services to these recipients the network has affiliated with the health departments, hospitals, and social service departments in each county.
The NCCN Mission Statement reads "We are an organized community network of healthcare providers and care managers, which partners with other community resources to improve the health and well-being of our Medicaid patients in an appropriate and cost-effective manner.”
Partnership for Community Care (P4CC) is responsible for the support of Medicaid patients in Guilford, Rockingham and Randolph counties. We presently support 70% of all Carolina Access patients in all three counties. Our purpose is to identify and minimize barriers to healthcare by helping primary care providers in our network to better utilize evidence-based disease state management guidelines. We also provide Care Management services to patients to reinforce what their provider's treatment plan is and to better manage their chronic conditions. These services are provided by medical and behavioral health social workers, nurses, pharmacists and a nutritionist. We also provide additional support to providers of children through our four Health Check Coordinators and our Early Intervention Specialist. Currently we have approximately 96,000 enrollees in our network.
Community Care of the Sandhills (CCS) is a regional partnership of primary care providers, hospitals, health departments, departments of social services and community organizations, including LMEs and AHECs. CCS is a non-profit organization that was formed as a 501 c3 in May 2003 to provide care management services to Carolina Access Medicaid recipients in Harnett, Hoke, Lee, Montgomery, Moore, Richmond and Scotland counties.
Located at 845 Church Street North in Concord, North Carolina, Community Care of Southern Piedmont serves the surrounding three-county area.
CC of SP is also one of only 17 sites nationally to be designated a Beacon Community, a federal grant program to promote the development of secure, private, and accurate systems electronic health records (EHRs) and health information exchanges. The Beacon Program aims to strengthen health information technology (health IT) infrastructure to improve care coordination, increase the quality of care, and slow the growth of health care spending.

Since 1998, CCWNC has partnered with local health care providers and the community to deliver high quality, patient-centered, and cost-effective care for Medicaid enrollees and other populations. From pregnancy to end-of-life, our initiatives address the health of the whole person, including medical, social, and mental health needs that impact quality of life.
Launched in 2003, Community Care of the Lower Cape Fear (CCLCF) serves the urban center of Wilmington, NC, and surrounding rural communities in Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties. CCLCF works in close collaboration with the physicians, hospitals, county health departments and departments of social services in our six-county network to improve health outcomes for nearly 98,000 eligible patients. Our 154 provider practices span from one of the very largest multi-specialty practices in the state to many single-provider, rural practices.
CCLCF is fortunate to participate in the federal CMS Multi-payor Advanced Primary Care Demonstration to extend our services to select Medicare, Blue Cross, and State Employee Health Plan patients in Bladen and Columbus Counties.
Launched in 2005, Carolina Collaborative Community Care (4C) is the 14th network to join Community Care of North Carolina and the only single county network. Cumberland County is home to Fort Bragg, the largest military installation in the United States, which provides for a very diverse and mobile population.
4C works in close collaboration with the physicians, hospital, county health department, department of social services and managed care organization in Cumberland County to improve health outcomes for approximately 62,100 eligible patients while reducing Medicaid costs. Our 90 provider practices span from a teaching multi-specialty practice to many single-provider, practices.
The Carolina Community Health Partnership Network serves the two rural counties: Cleveland and Rutherford. Of the 23,808 Medicaid enrollees served, almost half are children. There are two small community hospitals that have inpatient psychiatric admission units. The Network has a large population of enrollees diagnosed with Diabetes and Chronic Obstructive Pulmonary Disease. Of the 4090 enrollees in the Aged, Blind and Disabled category of Medicaid, 52% have a mental health diagnosis and two of our network’s primary care practices have a co-located mental health professional – one serving children and one serving adults.
Community Care of Wake and Johnston Counties encompasses two very different counties. Wake is a highly populated, largely urban county that contains our state capitol. Johnston County is a geographically large, rural county. Overall, our network has a high percentage of pediatric patients, both within our general population and our disabled population. We have a culturally and linguistically diverse population, with a high percentage of Spanish speaking families. Our network is made strong by active participation among our primary care providers, hospitals, public health agencies, mental health providers, social service agencies, and community partners.
Community Care Partners of Greater Mecklenburg (CCPGM) serves patients in the greater Charlotte area which includes Anson, Mecklenburg and Union counties. These counties represent a diverse demographic from very rural to very urban. In addition to working with nine local hospitals, CCPGM's care team interfaces with providers from two large hospital systems, Carolinas HealthCare System and Novant Health, as well independent practices and federally qualified heath centers. CCPGM's goal is to provide compassionate care coordination by utilizing an integrative team approach which includes nurse care managers, behavioral health specialists, pharmacists, social workers and Health Check coordinators.

Community Care Plan of Eastern Carolina is geographically the largest the Community Care of North Carolina networks. Covering 27 counties in Eastern North Carolina, most of the counties we serve are rural and many have long-standing access-to-care issues.
Counties served are Beaufort, Bertie, Camden, Carteret, Chowan, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Halifax, Hertford, Hyde, Jones, Lenoir, Martin, Nash, Northampton, Pamlico, Pasquotank, Perquimans, Pitt, Tyrrell, Washington and Wilson.
