Heart Health Now (HHN)
AHRQ has awarded seven implementation grants that use a variety of interventions to help small and medium-sized primary care practices build their capacity to use the ABCS of cardiovascular disease prevention:
- Aspirin in high-risk individuals
- Blood pressure control
- Cholesterol management, and
- Smoking cessation.
All of the implementation grants are testing the effectiveness of practice facilitation as a strategy to help primary care practices adopt the ABCS.

Advancing Heart Health in Primary Care is an initiative of the Agency for Healthcare Research and Quality (AHRQ). Its aim is to accelerate the dissemination and implementation of patient-centered outcomes research findings into primary care.
