CCNC's Transitional Care Success. Coverage continues of the CCNC study published in the August edition of Health Affairs detailing successful efforts to reduce readmissions for high-risk patients. See additional articles below. You can read the study via this page.
NC Tracks Update. NCTracks, the DHHS Medicaid claims system that went live in July, is still experiencing transitional difficulties. Recent coverage includes the articles below. There's also a Facebook page for providers' trying to navigate the system.

NEJM. In an article about prescription medicine nonadherence, The New England Journal of Medicine referenced CCNC’s successful work with physicians and clinical pharmacists in a care-coordinated effort. Read the full article here.
CHIPRA Evaluation Highlight. The August 2013 Evaluation Highlight from the national evaluation of the CHIPRA Quality Demonstration Grant program recognizes North Carolina as one of four states that is working with participating providers to enhance its ability to delivery unique health services and improve the overall quality of health care for adolescents. SE4A Award. CCNC and the NC Association of Area Agencies on Aging have been awarded the 2013 Corporate Partnership Award by the Southeastern Association of Area Agencies on Aging. The award will be presented at the Awards Luncheon on Monday, Sept. 23 in Orange Beach, Ala.
PCP Exam Passed. Congratulations to Jayson Caracciolo for earning the Project Management Institute’s Project Management Professional credential, the most important industry-recognized certification for project managers!
MAHEC Motivational Interviewing Training. The Mountain Area Health Education Center is hosting two Motivational Interviewing sessions to introduce participants to the theory, principles and skills of MI and related motivation-enhancing behavior change approaches. The first training is Sept. 12-13 in Asheville, and the second is Oct. 24-25 in Myrtle Beach. View the training session brochure here.
Making Strides Against Breast Cancer 5K. Join your CCNC co-workers, friends and family for Making Strides Against Breast Cancer, a 5K walk on Saturday, Oct. 5 at North Hills in Raleigh. Sign-up here to walk on the CCNC team or donate toward our $1,000 fundraising goal. The $25 registration fee benefits the American Cancer Society.
ACMA NC Conference. The American Case Management Association North Carolina Fall Conference will be held on Friday, October, 18, 2013 in Concord. For more information, view the brochure or register online here.
Project Lazarus Trainings. Training programs are scheduled for Hickory, Boone, Concord, Raleigh, Pinehurst and Reidsville in the coming months. Recovery Champion Award Nominations. Do you know individuals who have dedicated their talents, whether through professional work or volunteering, to successfully promote mental health and/or substance abuse recovery in NC? Recognize their contributions by nominating them for a Recovery Champion Award presented at the 5th Annual NC "ONE Community in Recovery" Conference.
Flu Shots. CCNC will hold an onsite flu shot clinic in the Central Office in October. Final dates and details will be announced when they are confirmed.
