This role will support the Practice Transformation Network (PTN) and PTN Leadership to provide centralized coaching efforts to drive movement through the five phases of transformation for assigned organizations and groups of practices. The Transformation Coach will assist in engagement and outreach activities as directed by PTN leadership to enroll eligible organizations and groups of practices. The Transformation Coach will ensure assigned organizations and groups of practices have access to and are able to implement CCNC population management tools such as: evidence-based best practice guidelines; quality improvement strategies around change management, systems engineering and rapid-cycles tests of change; care coordination and self-management; and how to use data and analytics to support transformation. The Transformation Coach will collaborate with the Practice Transformation QI Manager to ensure provision of a centralized coaching approach based on individual organizational structure that will support successful transformation within the PTN program. This position is anticipated to be primarily remote with travel in the field as needed to support transformation activities.

Essential Functions

  • Leverage teams within organizations or groups of practices to accomplish transformation through understanding team dynamics, forming, empowering and facilitating highly engaged and functional teams to accomplish goals in a busy environment
  • Utilize CCNCs QI Program Model to comply with practice transformation goals conveying a consistent, holistic approach to organizational change and accountability
  • Support and assist in engagement, recruitment and outreach activities as directed 
  • Educate on PTN CORE measure set, change package, tools, documentation, reporting and measurement requirements
  • Mentor, guide and provide centralized remote support to organizations and groups of practices in achieving transformation goals and proactively identifying opportunities for improvement; provide on-site support as deemed necessary
  • Provide support through the “five phases of transformation” and utilize baseline assessments as the starting point to develop aims and goals
  • Forge collaborative relationships to assist in practice assessment, transformation plan development and goal attainment during the phases of transformation
  • Serve as an active team member: learning from others, valuing all input and expertise, and establishing collaborative relationships with assigned organizations, groups of practices and the broader community itself
  • Promote and monitor strategies that contribute to drive the implementation of sustainable transformation activities
  • Provide ongoing training, support and technical assistance to ensure appropriate quality improvement approaches are being used to optimize results
  • Utilize and promote the methodologies and tools of continuous quality improvement
  • Seek out opportunities to promote staff knowledge and understanding of quality improvement and measurement
  • Support development & implementation of innovative strategies to spread effective change across organizations and groups of practices
  • Participate in QI trainings and collaborative activities including monthly calls and improvement effort analysis 
  • Facilitate development of strategies and best practices to drive improvement 
  • Support care coordination activities related to transformation goals and adoption of models of care enhancement
  • Ensure timely tracking and cataloging successes/challenges/barriers relating to Quality Improvement initiatives. Catalog PDSA cycles, trainings and resources in order to assist with the ‘spread’ of best practice initiatives.
  • Ensure timely completion of data collection, documentation and reporting requirements 
  • Serve as resource and educator on CCNC measures, applications and dashboards
  • Collaborate with and participate as a member of organizational or practice group QI teams as appropriate
  • Assist with analyzing and interpreting data to determine quality of care and to anticipate or quickly identify problems, patterns, and high-risk activities
  • Collaborate with local and state partners and stakeholders
  • Participate in all relevant CCNC trainings, conferences and meetings
  • Other duties as assigned


  • Proficient with IHI’s Model for Improvement & the use of PDSA cycles
  • Ability to evaluate Performance & Process Improvement methods and apply those best suited to achieving successful outcomes
  • Proficient with driver diagrams and alignment to goals and development of QI plans 
  • Ability to proactively and strategically prioritize multiple domains to ensure efficiency and effectiveness in practice transformation.
  • Proficient with practice or team facilitation and management of group dynamics
  • Proficient in facilitative interpersonal skills and communication strategies
  • Ability to serve as a change agent, motivate & stimulate innovative thinking
  • Understands the changing healthcare landscape and movement to alternative payment models such as MACRA/MIPS
  • Ability to apply theory into practice and be skillful in using appropriate tools at the appropriate times to attain multiple goals
  • Strong oral and written communications skills
  • Comfortable with public speaking & presentation
  • Proficiency in Microsoft Office. [Word, Excel, PowerPoint and Outlook]
  • Ability to work independently and as part of a team
  • Flexibility to adapt to shifting priorities and short-term deadlines
  • Strong organization and detail orientation skills
  • Data analysis, interpretation and graphing capability with ability to communicate results in both verbal and written formats in an appropriate manner base on target audience

Education and Experience

  •  Required Education/Experience: Education/Experience: Bachelor’s degree in Business Administration, Organizational Development, Nursing or other health related field combined with relevant quality improvement, outcomes, practice/team facilitation, and project management experience with demonstrated understanding of current trends in healthcare and proven experience using data to drive improvement; or an equivalent combination of education and experience which would provide the required knowledge, skills and abilities may be qualifying.
  •  Additional Education/Experience: At least 2 years of work experience with responsibilities in the following areas: 
    • Model for Improvement and rapid cycle change, data analysis systems analysis, graphical display of data/information, public speaking, group leadership and facilitation, and demonstrated problem solving and critical thinking skills. 
  • Experience working in or with a medical office or primary care setting highly preferred.
  • Experience with Meaningful Use, PQRS, PCMH Standards preferred
  • Experience working with EHRs or other Health Information Technology skills preferred
  • Experience working with Behavioral Health organizations or practices a plus

40 hours per week, with additional meetings on occasion as scheduled or required. Occasional flexibility with hours as required meeting the needs of clients/system. Travel required for identified needs.
