POSITION SUMMARY

The PTN Regional Coach Coordinator will support the Practice Transformation Network (PTN) efforts in their designated regions providing essential guidance and support to PTN coaches and collaborating with others participating in the PTN technical assistance model to provide a cohesive strategy to support practices in the transformation process. The PTN Regional Coach Coordinator will have a visible presence in the field and collaborate closely with the PTN coaches to monitor progress with practices, stay focused on PTN expectations and deliverables, provide training and technical assistance, help solve problems at individual practice sites and recommend strategies to drive improvement efforts. The PTN Regional Coach Coordinator provides a mechanism of continuous learning for PTN coaches through their expert knowledge of quality improvement, practice transformation, the changing healthcare landscape and new value based payment models. This position is anticipated to be primarily remote and in the field traveling within assigned regions of CCNC to support the coaches with training and transformation/quality improvement efforts with additional travel as needed to support PTN efforts.

Essential Functions

  • “Train the trainers” by functioning as coach teacher/mentor for the PTN Coaches
  • Assist with practice recruitment and outreach efforts
  • Assist in orientation of new PTN coaches that includes in the field support and development of curriculum materials 
  • Support the BHI and Pharmacy Medication Optimization technical assistance model and ensure collaboration with PTN coaches and alignment of efforts
  • Assist PTN coaches with developing and maintaining facilitative relationships with their practices (for example, positive relationships that advance the intervention and practice transformation process)
  • Assist PTN coaches in maintaining effective boundaries with their practices; this allows the coach to focus on capacity building rather than “doing for” the practice, but also permits hands-on support when appropriate 
  • Provide the content knowledge needed for practice transformation interventions 
  • Assist PTN coaches in forming and managing their quality team for a practice 
  • Assist PTN coaches in monitoring their panel of practices’ progress through the stages of the intervention model with implementation of change concepts and key drivers, and problem solving if barriers encountered
  • Assist PTN coaches with analysis and interpretation of multiple types of data to drive improvement and practice transformation
  • Support PTN coaches to maintain objectivity and not become so much a part of the practice system that they are no longer able to assist
  • Identify knowledge/skill gaps of coaching staff around emerging new initiatives and provide technical support/facilitate training to meet those needs such as QPP, QRUR
  • Develop, in concert with PTN Leadership, an ongoing orientation/maintenance training plan/curriculum for PTN coaches and other appropriate staff
  • Collaborate with the PTN Transformation QI Manager and Director Quality Improvement and Practice Support to facilitate all collaborative QI trainings and activities including monthly calls, ongoing improvement analysis, development of strategies and best practices to drive improvement in participating practices, and ensuring completion of all reporting requirements
  • Collaborate with organizational, network, provider and practice staff to facilitate engagement in quality improvement efforts and support the program quality improvement objectives through the provision and utilization of established QI methodologies, QI tools and data analysis
  • Collaborate with PTN Leadership around activities related to PTN tracking and key deliverables, web portal functionality, access to PTN tools, dashboard reports, coach folder documents and compilation of data for various reports and requests 
  • Participate in planning efforts for overall health care reform moving towards a value based payment system
  • Promote leveraging teams within practices to accomplish transformation through understanding team dynamics, forming, empowering and facilitating highly engaged and functional teams to accomplish goals in a busy environment
  • Utilize CCNCs QI Program Model to comply with practice transformation goals conveying a consistent, holistic approach to organizational change and accountability
  • Provide ongoing feedback to PTN Leadership regarding successes, challenges/barriers, opportunities for engagement

Requirements

  • Proficient with IHI’s Model for Improvement, PDSA cycles and Lean QI Methodology
  • Strong understanding of the changing healthcare landscape and movement to alternative payment models such as MACRA/MIPS
  • Ability to evaluate Performance & Process Improvement methods and apply those best suited to achieving successful outcomes
  • Proficient with driver diagrams and alignment to goals and development of QI plans
  • Expert practice facilitator 
  • Ability to proactively and strategically prioritize multiple domains to ensure efficiency and effectiveness in practice transformation.
  • Ability to serve as a change agent, motivate & stimulate innovative thinking
  • Ability to apply theory into practice and be skillful in using appropriate tools at the appropriate times to attain multiple goals
  • Skilled in team facilitation & group process
  • Strong oral and written communications skills
  • Comfortable with public speaking & presentation
  • Proficiency in Microsoft Office. [Word, Excel, PowerPoint and Outlook]
  • Ability to work independently and as part of a team
  • Flexibility to adapt to shifting priorities and short-term deadlines
  • Strong organization and detail orientation skills
  • Data analysis, interpretation and graphing capability with ability to communicate results in both verbal and written formats in an appropriate manner base on target audience

Education and Experience

  • Education/Experience: Bachelor’s degree in Business Administration, Organizational Development, Nursing or other health related field combined with relevant, progressive quality improvement work experience in a health care setting; or an equivalent combination of education and experience which would provide the required knowledge, skills and abilities may be qualifying. Extensive experience in practice facilitation. Expert knowledge of landscape of healthcare reform and value based payment models: MACRA, MIPS, APMs
  • Additional Education/Experience: At least 4 years of work experience with responsibilities in the following areas: process improvement, systems analysis, graphical display of information, report writing, public speaking, group leadership and facilitation, and demonstrated problem solving skills.
  • Experience working in the primary care setting preferred.
  • Experience/technical assistance experience with electronic health records; data collection, data entry and reporting preferred
  • Quality certification preferred: CPHQ, Lean Six Sigma; PCMH-CCE

Working Conditions

40 hours per week, with additional meetings on occasion as scheduled or required. Occasional flexibility with hours as required meeting the needs of clients/system. Travel required for identified need
