The CCNC Toolkit
CCNC's "How To" Guide
With the support of the Commonwealth Fund, CCNC has launched a new website dedicated to helping others learn from CCNC's experience. We are happy to share what we have learned in the last decade about "best practices" in population-based health management and building nonprofit networks.
The new site provides detailed information on how CCNC's nonprofit networks function and the types of health care professionals who collaborate in our system. You can also download a variety of documents, educational tools and measurement criteria.
